Educational Opportunity Program (EOP)

 

Student Contract of Responsibilities for Transfers--Fall 2009


The Educational Opportunity Program (EOP) at Cal Poly, San Luis Obispo is committed to our partnership with you so that your educational experience will be a successful one.  To achieve this goal, there are several commitments we will make with you.  Please read this contract, check “I accept” and submit online by May 1, 2009.  We look forward to working with you!

 

You are required to do the following:

 

1.

Send your updated transcripts to Cal Poly Admissions (NOT EOP) to verify your completion of GE requirements in English and math.  If you have already sent them, do not send again.   Refer to your Cal Poly Welcome Information at http://www.ess.calpoly.edu/_admiss/undergrad/newly_admitted.html and/or the Cal Poly Catalog for English Placement Test (EPT) and Entry Level Math (ELM) exemptions for most transfer students.  You must take these exams in May at any CSU campus if you are not exempt.


2.

Attend the Cal Poly SOAR Program (Student Orientation, Advising & Registration) and EOP meetings at SOAR, scheduled in July and August; you will receive an invitation later this spring.


3.

Enroll and complete the following in order to keep your SEOP Grant eligibility for Winter and Spring Quarters, based on your Financial Aid eligibility:

  • First Year Seminars during Fall Quarter 2009
    You will have the opportunity to participate in an interactive seminar series sponsored by EOP that includes key topics:  time management, college policies, career planning, study skills, university transition and more.  Before Fall Quarter, you’ll receive information on how you can fulfill this part of your contract. 
  • A Supplemental Workshop in Math or Science
    If you still have to complete any of the courses included in these programs, workshops will be built into your fall schedule when Cal Poly provides it to you.

 


4.

Complete a minimum of 12 units each quarter that you are enrolled at Cal Poly. To continue your SEOP Grant eligibility, 12 units are required each quarter.


5.

Maintain your CURRENT email, mailing address and other contact information on-line through http://my.calpoly.edu on the Personal Information tab.  This is extremely important


6.

Meet with your EOP advisor regarding:

  • Course scheduling each quarter
  • Withdrawing from school or class
  • Applying to another college campus
  • Academic difficulty

  • Financial difficulty
  • Personal advising
  • Change of major
  • Summer employment
  • Career goals

I have read the above contract and understand my responsibilities to EOP.  I understand the conditions of this contract will be explained in more detail at the Educational Opportunity Program meeting during SOAR prior to my first quarter at Cal Poly.  I understand that I will lose my SEOP Grant if I do not complete the terms of this contract.  Marking this box will serve as my electronic signature.

I have read the above contract and do not accept my EOP eligibility at Cal Poly; I realize this may rescind my admission acceptance from Cal Poly and will delete my EOP grant.
