Educational Opportunity Program (EOP)

 

Student Contract of Responsibilities for Freshmen--Fall 2009


The Educational Opportunity Program (EOP) at Cal Poly, San Luis Obispo is committed to our partnership with you so that your educational experience will be a successful one.  To achieve this goal, there are several commitments we will make with you.  Please read this contract, check “I accept” and submit online by May 1, 2009.  We look forward to working with you!

 

You are required to do the following:

 

1.

Take the English Placement Test (EPT) and the Entry Level Mathematics (ELM) exam during May

EPT exemptions: http://writingskills.calpoly.edu/ept/index.html#exempt
ELM exemptions: http://math.calpoly.edu/elm.html
You must take these exams by the end of May at any CSU campus unless you are exempt.


2.

Apply for and attend the Summer Institute Program. Summer Institute is an exciting summer bridge program and information will be sent to you soon. Summer Institute runs from Aug. 2-25, 2009 and EOP freshmen receive priority.  Please note that because EOP expects you to attend Summer Institute, the cost is covered by Cal Poly EOP and Financial Aid as long as your 2008-09 FAFSA is completed by the program deadline (see #3 below).


3.

Complete your 2008-09 FAFSA (based on 2007 taxes) as part of your Summer Institute application. This FAFSA is in addition to the 2009-10 FAFSA you completed for this coming academic year that was based on 2008 taxes.  Therefore, in order to be selected for Summer Institute, your 2008-09 FAFSA is also required.


4.

Attend the SOAR Program (Student Orientation, Advising & Registration). You will attend the SOAR Program during Summer Institute. You don’t need to register for SOAR separately, since you will participate in this program as part of Summer Institute.


5.

Week of Welcome (WOW) at Cal Poly is also a great experience but is not required; EOP meetings are scheduled during WOW for those who attend WOW.  WOW program information will be sent to you in August.


6.

Enroll and complete the following in order to keep your SEOP Grant eligibility.  You must complete these items in order to continue your SEOP grant for Winter and Spring Quarters, based on your Financial Aid eligibility

  • First Year Seminars during Fall Quarter 2009
    You will have the opportunity to participate in an interactive seminar series sponsored by EOP that includes key topics:  time management, college policies, career planning, study skills, university transition and more.  Before Fall Quarter, you’ll receive information on how you can fulfill this part of your contract 
  • A Supplemental Workshop in Math or Science  based on your SAT (and ELM, if required) score. These will be built into your fall schedule when Cal Poly provides it to you.
  • An English composition or preparation course according to your EPT and SAT score and EOP advising: ENGL 101/102, 103, 111/112, or 134.   These will be built into your fall schedule when Cal Poly provides it to you.

7.

Complete a minimum of 12 units each quarter that you are enrolled at Cal Poly. To continue your SEOP grant eligibility, 12 units are required each quarter.


8.

Maintain your CURRENT email, mailing address and other contact information on-line through http://my.calpoly.edu on the Personal Information tab.  This is extremely important!


9.

Meet with your EOP advisor regarding:

  • Course scheduling each quarter
  • Withdrawing from school or class
  • Applying to another college campus
  • Academic difficulty

  • Financial difficulty
  • Personal advising
  • Change of major
  • Summer employment
  • Career goals

I have read this contract and understand my responsibilities to EOP.  I understand the conditions of this contract will be explained in more detail at the Educational Opportunity Program meeting during Summer Institute and SOAR (prior to my first quarter at Cal Poly).  I understand that I will lose my SEOP Grant if I do not complete the terms of this contract.  Marking this box will serve as my electronic signature.

I have read the above contract and do not accept my EOP eligibility at Cal Poly; I realize this may rescind my admission acceptance from Cal Poly and will delete my EOP grant.
